Most of our patients choose to be cared for in the comfort of their own home and we are here to help you do that

Through our specialist community services team and our 24-hour telephone Care Advice Line, we ensure, wherever possible, that you can receive vital care and advice when and where you need it most, including at care homes.

What to expect

When we receive your referral, we will call you to arrange for one of our nurses to contact you by phone, video call or a visit.

During this conversation, we will review your medications, discuss any symptoms you may be experiencing, talk to your family, friends and carers if they’re around, and make a plan for your care with you.

As a specialist palliative care service, we work closely alongside your GP, hospital team and other NHS services. We do not replace these services but we can offer advice and support to them to help them to support you. Sometimes we ask other healthcare professionals to visit you and provide care following our advice.

Our team provides 24/7 support, from managing prescriptions and coordinating medical appointments to providing personal care and emotional support, we work closely with NHS and community providers to help you stay in your preferred place of care and avoid unnecessary hospital visits.

Our 24-hour adult care advice line offers access to coordinators and qualified nurses for advice, reassurance, and personalised practical support whenever you need it.

Support in your community

We also offer a range of supportive groups, courses and activities to all members of the community, that aim to provide a supportive and friendly atmosphere to explore your experiences and learn more about them.

You can attend courses about topics such as cancer support, eating and nutrition, breathlessness, carer support, and understanding loss; groups that help you talk with others; or take part activities.

Our groups are online and in person. They are also a mix of drop in and bookable.
