What does the My Care Co-ordination Team do?

Our 24-hour advice and support line is for you, your family, carers and professionals, to help you stay at home to receive care, if you choose. By working closely with the local paramedic services, out-of-hours GPs, community services and the hospital, our team can prevent unnecessary hospital visits and stays. The team also works with hospitals to help get people home at the end of life.

How can the My Care Co-ordination Team help me?

We make sure your care is co-ordinated for the last two years of life. Meet the team who are part of your care.

We can organise palliative-care support workers to visit you in the community, at home, and in nursing and residential homes, offering personal care, practical help and emotional support to you and your family. We can chase prescriptions, cancel and make medical appointments and inform those involved with your care of any changes.


Who do you work with?

The My Care Co-ordination Team works closely with community nursing services from Luton and Bedfordshire, Macmillan nurses, Luton and Dunstable University Hospital, general practitioners, the East of England Ambulance Service and social services.


How do I get support?
