The My Care Co-ordination Team at Keech Hospice Care is thrilled to be one of three finalists for an award in Effective Co-ordination of Care. The My Care Team supports Luton patients with long-term or end-of-life conditions who want their place of care to be in their own homes. This means working with many local services to make sure patients and their families can remain at home with all the help they need. The awards, which are organised by the National Council for Palliative Care, were announced in a ceremony at the House of Commons on 22nd February. Congratulations to the My Care Co-ordination Team on achieving such a fantastic recognition.
Sarah Myford, MCCT Clinical Lead Nurse, said “I can not explain how proud I am of the team, past and present. Everyone’s passion and belief in the work we are doing is what continues to drive us forward.
